1. ACKNOWLEDGMENT OF SERVICE

I understand that I am receiving a sunless spray tanning service, which applies a cosmetic solution to the skin to create a temporary bronzed appearance.

I acknowledge:

• Results vary based on skin type, preparation, and aftercare

• The solution does NOT contain sunscreen and does not protect against UV exposure

• The technician cannot guarantee a specific shade or outcome

2. PRE-TAN RESPONSIBILITY (CLIENT INITIALS: ______)

I confirm that I have been advised to:

• Shower, exfoliate, and shave prior to appointment

• Avoid lotions, oils, perfumes, deodorant, and makeup

• Wear loose, dark clothing after appointment

I understand failure to follow prep instructions may negatively affect results.

3. AFTERCARE RESPONSIBILITY (CLIENT INITIALS: ______)

I agree to:

• Avoid water, sweating, or skin contact for 8–12 hours (or as directed)

• Avoid tight clothing during development period

• Moisturize daily after first rinse

• Avoid exfoliation, chlorine, and long baths

I understand improper aftercare may cause:

• Uneven fading

• Shortened tan lifespan

• Streaking or patchiness

4. HEALTH & ALLERGY DISCLOSURE

I understand:

• A patch test is available upon request

5. LIABILITY WAIVER

I voluntarily assume all risks associated with this service.

I release and hold harmless:
BTAN, LLC, Brandon Busche, and any affiliates
from any claims related to:

• Allergic reactions

• Skin discoloration or dissatisfaction

• Staining of clothing, fabrics, or personal property

6. STAINING ACKNOWLEDGMENT

I understand that spray tanning solution may temporarily stain:

• Clothing

• Bedding

• Skin (palms/soles if not properly maintained)

I accept full responsibility for any such occurrences.

7. PROFESSIONAL STANDARDS

I acknowledge that:

• Services are performed in a professional, respectful, and discreet manner

• I may request adjustments to comfort level at any time

• I am receiving service from a trained professional with extensive experience

8. NO GUARANTEE POLICY

Due to the nature of sunless tanning:

• No refunds are provided based on color outcome

• Adjustments may be offered at technician discretion

9. PHOTO / MARKETING RELEASE (OPTIONAL)
